Please make sure to use a separate registration form for each den.

Medical Forms:
Each youth and adult should have a completed medical form on file at daycamp. Adults and youth that do not have a fully completed medical form will not be allowed in camp.
A completed medical form must include:
- A current copy of medical insurance. If none, it must state none.
- A listing of current and up-to date immunizations. If no immunizations, you must fill out an immunization exempt form for approval.
- Parents signature
- Information completed in full
Did you know that each pack is responsible for having a copy of this medical form year around to have available in case of an emergency during meetings, events, and outings?
